The Case Manager will provide professional clinical care to patients with behavioral healthcare needs and work with clinical and case management staff to coordinate care and assist with treatment plans for clients.

Responsibilities:
Participate in the assessments and developments of the treatment plan.
Implement services for patients in accordance with their individualized treatment plan (ITP).
Collaborate with the patient and patient's family or significant others to implement an effective treatment plan, explaining the available clinical options to the team, including the advantages and disadvantages of each option.
Communicate and document patients' progress toward their recovery goals.
Maintain the patient's comprehensive clinical record, including documentation of activities performed as part of the service delivery process (e.
g.
, assessments; provision of services; coordination of care; discharge planning).
Work collaboratively with the clinical team to engage, educate, communicate, and coordinate care with patients, their family, behavioral and general medical and dental health care providers, community resources, and others to ensure that all services prescribed in the ISP are implemented.
Provide assistance in maintaining, monitoring, and modifying covered behavioral health services.
Seek out necessary resources other than covered services to meet basic needs.
Ensure all appropriate referrals for identified services on the service plan are made and coordinated with service providers via contracted network providers or community resources.
Ensure all covered services identified on the treatment plan are evaluated and updated monthly.

Qualifications:
Bachelor's Degree in a clinical/mental health field or equivalent combination of education, training, and/or experience.
At least 1 year of experience in behavioral healthcare or substance abuse settings serving culturally diverse populations.
Knowledge of health care, the detoxification process, addiction, co-occurring disorders, DSM criteria, and health care terminology.
Able to read and interpret written information; write clearly and informatively; edit work for spelling and grammar; speak clearly and persuasively in positive or negative situations; listen and gain clarification; respond well to questions; demonstrate group presentation skills; participate in meetings.
Excellent organizational skills with the ability to prioritize workload and meet deadlines.
Able to work in a fast-paced environment and juggle multiple competing tasks and demands.
Strong knowledge of a variety of computer software applications in word processing, spreadsheets, and presentations (MS Word, Excel, and PowerPoint).
Familiarity with diverse cultures and gender-specific issues and the ability to incorporate the needs of gender and culturally diverse groups into the organization.
Able to maintain appropriate confidentiality at all times when interacting with patients, residents, families, visitors, referral sources, and all other contacts.
Excellent customer relation skills as evidenced by supportive and constructive communication with all contacts including coworkers, patients, residents, visitors, families, and referral sources.
Work Environment:
This job operates in a professional office environment.
This role routinely uses standard office equipment such as computers, phones, photocopiers, and filing cabinets.
